Please give us as much information about your enrollment as possible, so we can make sure we send you the correct transcript. Name and social security number are usually sufficient. Please also tell us where to send your transcript.

If you wish to expedite the process and are paying by credit card, you may fax your request to (408) 725-8146. The fax machine is secure and your information is shredded once your request is processed. Your credit card number and expiration date are required, but not the 3 number CVC.
